BackgroundHealth decisions occur in a context with omnipresent social influences. Information concerning what other patients decide may present certain interventions as more desirable than others.ObjectivesTo explore how physicians refer to what other people decide in conversations about the relevancy of cardio-pulmonary resuscitation or do-not-attempt-resuscitation orders.MethodsWe recorded forty-three physician–patient admission interviews taking place in a hospital in French-speaking Switzerland, during which CPR is discussed. Data was analysed with conversation analysis.ResultsReference to what other people decide in regards to CPR is used five (...) times, through reported speech. The reference is generic, and employed as a resource to deal with trouble encountered with the patient’s preference, either because it is absent or potentially incompatible with the medical recommendation. In our data, it is a way for physicians to present decisional paths and to steer towards the relevancy of DNAR orders. By calling out to a sense of membership, it builds towards the patient embracing norms that are associated with a desirable or relevant social group.ConclusionsIntroducing DNAR decisions in terms of what other people opt for is a way for physicians to bring up the eventuality of allowing natural death in a less overt way. Formulating treatment choices in terms of what other people do has implications in terms of supporting autonomous and informed decision making, since it nudges patients towards conformity with what is presented as the most preferable choice on the basis of social norms. In this paper, the principal objections to unconditional forgiveness are canvassed, primarily that it fails to take wrongdoing seriously enough, and that it displays a lack of self-respect. It is argued that these objections stem from a mistaken understanding of what forgiveness actually involves, including the erroneous view that forgiveness involves some degree of condoning of the offence, and is incompatible with blaming the offender or punishing him. Two positive reasons for endorsing unconditional forgiveness are considered: respect for persons and (...) human solidarity; and it is argued that the latter provides more plausible grounds for it than the former. This book offers a new approach to the analysis of the multiple meanings of English modals, conjunctions, conditionals, and perception verbs. Although such ambiguities cannot easily be accounted for by feature-analyses of word meaning, Eve Sweetser's argument shows that they can be analyzed both readily and systematically. Meaning relationships in general cannot be understood independently of human cognitive structure, including the metaphorical and cultural aspects of that structure. Sweetser shows that both lexical polysemy and pragmatic ambiguity are shaped by our (...) metaphorical folk understanding of epistemic processes and of speech interaction. Similar regularities can be shown to structure the contrast among root, epistemic and speech act uses of modal verbs, multiple uses of conjunctions and conditionals, and certain processes of historical change observed in Indo-European languages. Since polysemy is typically the intermediate step in semantic change, the same regularities observable in polysemy can be extended to an analysis of semantic change. (shrink)

This article analyzes the institutionalization of the global organic agriculture field and sheds new light on the conventionalization debate. The institutions that shape the field form a tripartite standards regime of governance that links standard-setting, certification, and accreditation activities, in a layering of markets for services that are additional to the market for certified organic products. At each of the three poles of the TSR, i.e., for standard-setting, certification, and accreditation, we describe how the corresponding markets were constructed over time (...) and the role of the different actors in their evolution. We analyze the politics at stake among the actors at each pole, their competing or cooperative interests and visions, and the tensions between them in the promotion of markets. Through the lens of the TSR heuristic, we show that the institutionalization of the organic field beginning in the 1990s and its de facto inclusion in the broader sustainability field beginning in the 2000s contribute to a progressive distancing between the organic movement and its initial political project of alterity, to which public and private actors both contribute actively. As a set of interlinked market institutions, the TSR orients and narrows the scope of debate, which becomes restricted to “market-compatible” dimensions and objects. We conclude that the TSR is a promising heuristic for analyzing contemporary global regulation. (shrink)

This essay focuses on conceptual engineers who aim to improve other people's patterns of inference and attention by shaping their concepts. Such conceptual engineers sometimes engage in a form of epistemic paternalism that I call paternalistic cognitive engineering: instead of explicitly persuading, informing and educating others, the engineers non-consultatively rely on assumptions about the target agents’ cognitive systems to improve their belief forming. The target agents could reasonably regard such benevolent exercises of control as violating their sovereignty over their own (...) belief formation. This is a pro tanto reason against such engineering. In addition to the relevant projects of conceptual engineering, paternalistic cognitive engineering plausibly includes certain kinds of nudging and evidence suppression. I distinguish the sovereignty-based concern from other ethical worries about conceptual engineering and discuss how one might justify the relevant conceptual engineering projects despite the sovereignty-based reason against them. This South African study investigates the lived experiences of a group of isiZulu mothers of children diagnosed with multiple disabilities. Data collection from regular focus group discussions proceeded with the assistance of a translator skilled in working in isiZulu and English. The phenomenological approach employed revealed the mothers' philosophical acceptance of their child's disability. Issues of concern to the women that emerged include the effects of the child's disability on their lives, the treatment options for their children, and their perceptions (...) of the causes of the disability. The women reflect on both traditional and Western treatment options and articulate the constraints they experience as caregivers, with limited child-care facilities preventing them from finding employment, difficulties in accessing social service grants exacerbating their position, and various levels of family and community support experienced. The study underlines the need for more adequate service provision for children with special needs and their families, and for intervention programmes to be informed by an understanding of the contexts and ways of making meaning of those whose needs they are intended to serve. Abstract“Science Is Politics By Other Means” (SIPBOM) was coined in The Pasteurization of France, Latour’s 1984 empirical study of the birth of microbiology. Yet, it encapsulates an outstanding political theory of science that Latour has never formalized and that has remained unnoticed to this day. The theory is comprised of two dimensions. The first one is the ontological labor performed by science, that is, the laboratory production of new nonhumans. The second one is the ability of science to devise and (...) implement novel policies targeted at the new beings it produces. These “other means” are incorporated in political projects and contribute to the shaping of society. Fifteen years later, Latour published Politics of Nature ([1999] 2004), a full-blown political treatise equally devoted to the political character of science. It would be mistaken, however, to assume that it falls in the same SIPBOM paradigm as the Pasteur study. The compositionist theory it offers redefines politics as the institution of the nonhumans that make up external reality, a task that has traditionally been monopolized by Science. In this sense, “science is politics by other means” has become “politics is science by other means,” these “other means” now referring to “cosmopolitics,” that is, the due process advocated by compositionism. The first claim of the present paper is that the respective weight ascribed to politics and ontology is different in The Pasteurization of France and in Politics of Nature. The second claim is that compositionism is not as successful as Latour’s early theory to account for the politicity of science. (shrink)

Some of the virtues have a very stable place in our understanding of goodness – beneficence and courage are unlikely ever to lose their high standing. But other virtues have something like a life cycle: they move from a marginal status to to a central one, and sometimes they move back again to the margins, or even beyond the domain of virtue altogether. Chastity is one example of this; humility is another. There was a period in which humility wasn’t a (...) virtue at all (see Aristotle on the great-souled man); with the rise of Christianity (blessed are the meek) it became, not a cardinal, but still a central, virtue, though possibly more honoured in the breach than in the observance; now in the post-Christian world it seems, if a virtue at all, a somewhat creepy one. Why is this – what is it about humility that made it so important during the Christian period, and now makes us so ambivalent about it? And is there any way of rescuing it for a secular context, so that we can once again wholeheartedly endorse and admire it as one of the virtues? (shrink)

How far can we ever hope to understand the Holocaust? What can we reasonably say about right and wrong, moral responsibility, praise and blame, in a world where ordinary reasons seem to be excluded? In the century of Nazism, ethical writing in English had much more to say about the meaning of the word `good` than about the material reality of evil. This book seeks to redress the balance at the start of a new century. Despite intense interest in the (...) Holocaust, there has been relatively little exploration of it by philosophers in the analytic tradition. Although ethical writers often refer to Nazism as a touchstone example of evil, and use it as a case by which moral theorising can be tested, they rarely analyse what evil amounts to, or address the substantive moral questions raised by the Holocaust itself. This book draws together new work by leading moral philosophers to present a wide range of perspectives on the Holocaust. Contributors focus on particular themes of central importance, including: moral responsibility for genocide; the moral uniqueness of the Holocaust; responding to extreme evil; the role of ideology; the moral psychology of perpetrators and victims of genocide; forgiveness and the Holocaust; and the impact of the `Final Solution` on subsequent culture. Forgiveness usually gets a very good press in our culture: we are deluged with self-help books and television shows all delivering the same message, that forgiveness is good for everyone, and is always the right thing to do. But those who have suffered seriously at the hands of others often and rightly feel that this boosterism about forgiveness is glib and facile. Perhaps forgiveness is not always desirable, especially where the wrongdoing is terrible or the wrongdoer unrepentant. In this book, (...) Garrard and McNaughton suggest that the whole debate suffers from a crippling lack of clarity about what forgiveness really amounts to. They argue that it is more difficult, complex and troubling than many of its advocates suppose. Nevertheless, they conclude, a proper understanding of forgiveness allows us to avoid cheap and shallow forms of it, and enables us to see why it is right and admirable to forgive even unrepentant wrongdoers. On the day on which Dr Harold Shipman, the Manchester serial killer, was convicted, there was wall-to-wall coverage of it in the media. During the course of one of the many reports, the daughter of one of his victims was interviewed, and asked for her views on why Shipman had acted as he did. What she said was this: she’d tried and tried to understand or explain his deeds, and she could only come to the conclusion that he was a (...) really evil man. In saying this, she clearly meant to convey more than the obvious truth that what Shipman did was very wrong and entirely to be condemned. It seems to me that a secular as well as a religious audience would find the daughter’s appeal to evil as an appropriate explanation perfectly comprehensible. But what exactly was it that was being attributed to Shipman, and how was it thought to explain his actions? (shrink)

We also help everyone involved with the care of seriously ill patients focus on what is best based on the values of the most important person in the room; the one in the hospital bed. So, when we call for (...) a bioethics consultation from a clinical ethicist, it’s because something has gotten stuck or, perhaps, we could benefit from a reframe—or a new set of eyes to ground the scenario or the emotional miasma that tends to linger and drift when on the cusp between life and death.In the 1990s, at least in my medical school and residency programs, medical ethicists in the hospital were not often a part of direct patient care. They were like philosophers, available to curbside and advise on ethically murky cases but at a distance from the trenches. These wise, generally older men with white beards parceled out all potential ethical sides of the case, leaving us to decide the best way forward. The Socratic Method1 was often employed, so we were left to grapple with the ambiguity that exists in most issues about what’s right or wrong. It sometimes felt frustrating but now I realize the importance of honing the skill of tolerating the feeling of not immediately knowing the best way to proceed in the practice of medicine. As it turned out, by the time I started practicing palliative care in 2013 and began to regularly collaborate with what was then our clinical ethics partners in care, our ethics director Dr. F was also a wise older man with a white beard. But he and the rest of his (non-male, un-bearded) team didn’t have that untouchable ivory tower kind of protection around them. They were accessible. They dug into the hard stuff right alongside us when they were asked to get involved.One time when I consulted ethics on a case, I’d been navigating a switch from practicing internal medicine to palliative care. At the time I was dealing with personal ambivalence—feeling mired in the heaviness that infused my workdays and unsure if I could take it. It turned out that a clinical consultation, and a family meeting with Dr. F, helped in more ways than one.That day, we gathered in the medical intensive care unit conference room. It doubled as a storage closet, so the team, including our patient’s family, dragged in chairs and stools and crowded around the oblong melamine table next to the whiteboard with half-erased arterial blood gas calculations. We hunched together to see where things stood and make some essential decisions.As we do in family meetings, we reconfirmed our roles on the team: ICU fellow, resident, and medical student. Nurses fluttered in and out as their patient care allowed. The patient’s daughter with silver-streaked hair was in attendance, as well as her son, who was slumped in his seat. The jury remains out about the bottom-line relevance of organisational spirituality. This article reviews the arguments made thus far, using those sources most commonly cited as providing ‹evidence’ that organisational spirituality adds value to the bottom line. Having collated the evidence, this article offers some observation about the robustness of this existing ‹business case’. It then offers some preliminary conclusions on the literature review, examining the merits of pursuing a ‹business case’ in this field and identifying some specific questions for (...) future research. Aristotle’s writings contain more direct statements about priorities and rankings among the various sciences, degrees of accuracy within them, routes to knowledge from first principles, “first philosophy” and its characteristics, and the relation between sciences and practical concerns than almost any other philosopher we know.Yet taken together, Aristotle’s statements on these matters belie the apparent systematicity of his philosophical temperament. Almost every devotee of Aristotle is compelled to choose certain texts as authoritative and relegate others to some specific topic-context in (...) which they have limited validity. An only slightly shaky consensus among Aristotle’s readers has emerged through this process over time: on that consensus, Aristotelian first philosophy equals what later came to be called metaphysics, and is characterized by its remoteness from particular human discourses and desires, and from the types of knowledge we connect most directly with perception. First principles are also divorced from perception and are known through an incompletely explained intuitive process, but remain indemonstrable. Theoretical wisdom or contemplation soars above the domain of the human struggle, providing a stabilizing but distanced and abstract mode of considering the highest and most divine matters of which we are capable of thinking. In this article, I present my view that only through value conflicts can we become aware of the relevant values influencing science. The discussion about values in science has been ongoing for the past decades; unfortunately, a crucial part of the discussion—value awareness—has not been addressed. This article tries to bridge this gap by focusing on the problem of value recognition through analyzing several cases of value conflicts. I will show that unless some value conflict occurs, the values influencing science (...) remain invisible to us. This means that to discuss the relevant influences of values in science, we need to focus on value conflicts occurring in science. Only then can we recognize the values influencing science and have a meaningful debate about them. This article is the first foray into the issue of value recognition and can help clarify some unaddressed issues in the values in science debate. (shrink)

There seems to be something self-evident—irresistibly so, to judge from its gleeful propagation—about the use of the phrase, “Jane Austen and the Masturbating Girl,” as the Q.E.D. of phobic narratives about the degeneracy of academic discourse in the humanities. But what? The narrative link between masturbation itself and degeneracy, though a staple of pre-1920s medical and racial science, no longer has any respectable currency. To the contrary: modern views of masturbation tend to place it firmly in the framework of optimistic, (...) hygienic narratives of all-too-normative individual development. When Jane E. Brody, in a recent “Personal Health” column in the NewYork Times, reassures her readers that, according to experts, it is actually entirely possible for people to be healthy without masturbating; “that the practice is not essential to normal development and that no one who thinks it is wrong or sinful should feel he or she must try it”; and that even “’those who have not masturbated … can have perfectly normal sex lives as adults,’” the all but perfectly normal Victorianist may be forgiven for feeling just a little—out of breath.3 In this altered context, the self-evidence of a polemical link between autoeroticism and narratives of wholesale degeneracy 4 draws on a very widely discredited body of psychiatric and eugenic expertise whose only direct historical continuity with late twentieth-century thought has been routed straight through the rhetoric and practice of fascism. But it now draws on this body of expertise under the more acceptable gloss of the modern, trivializing, hygienic-developmental discourse, according to which autoeroticism not only is funny—any sexuality of any power is likely to hover near the threshold of hilarity—but also must be relegated to the inarticulable space of infantility. 3. I will defend explication, in a Carnapian sense, as a strategy for revisionary ontologists and radical sceptics. The idea is that these revisionary philosophers should explicitly commit to using expressions like “S knows that p” and “Fs exist” differently from how these expressions are used in everyday contexts. I will first motivate this commitment for these revisionary philosophers. Then, I will address the main worries that arise for this strategy: the unintelligibility worry and the topic shift worry. I will focus (...) on the latter worry and provide a solution that makes use of a distinction between practically and theoretically oriented beliefs. On my view, the revisionary philosophers who admit to departing from the everyday language can still criticize everyday knowledge and existence claims, by arguing that while the language embedded in these claims is suitable for beliefs-1, it is not suitable for beliefs-2. This article examines a particular shipboard newspaper situated within the centuries- long hunt for the Northwest Passage. The newspaper existed in both an original handwritten form produced on a ship in the Arctic and as a printed edition in London. An examination of the newspaper in both versions suggests the ways in which the same text can be transformed by variations on its physical form, its readers, and its temporal situation. This study shows the ways that a focus on print (...) culture and books as objects can be used to interpret primary historical sources, revealing the value of dealing with primary sources in a personal way. Surprisingly, when Laurence Sterne’s Yorick sets his head toward Dover, it is with no developed motive of connoisseurship or curiosity: the gentleman dandy ups with his portmanteau at the merest glance of “civil triumph” from a male servant. Perhaps we are in the world of P. G. Wodehouse, with a gentleman’s gentleman who happens, like Jeeves, to be the embodiment of all the prescriptive and opportunistic shrewdness necessary to maintain his master’s innocent privileges—but it is impossible to tell; the servant (...) utters his five words, glances his glance, and disappears from the novel. The prestige that has lent force to his misprision—or is it a sneer?—seems to belong not to a particular personality but to a position, a function , a bond between gentleman and gentleman’s gentleman that, throughout this novel, makes up in affective and class significance what it lacks in utilitarian sense. Yorick’s bond to another valet is the most sustained and one of the fondest in the novel; and, for most of the novel, the bond is articulated through various forms of the conquest and exchange of women.In the discussion ahead, I will be using the “exchange of women” paradigm taken from Claude Lévi-Strauss and, for example, René Girard and Gayle Rubin, to focus on the changing meanings of the bonds between men in a seventeenth-century play and an eighteenth-century novel.1 These discussions are part of a book-length study of what I am calling “male homosocial desire”—the whole spectrum of bonds between men, including friendship, mentorship, rivalry, institutional subordination, homosexual genitality, and economic exchange—within which the various forms of the traffic in women take place.
